Early Release Days 2011-2012 School Year

Medication Consent Form

      - Must be filled out BEFORE any medication is issued, whether routine or occasional, ie. EpiPens, Antibiotics, etc.

Individual Heathcare Form

      - For children with chronic illness. Must be filled out and kept on site daily. In the case of EpiPens, 2 EpiPens must be kept on site at all times.

Off-Site Permission Form

       - Must be filled out and signed 1 week in advance of a field trip.
